For what programs is an FMS needed? 

The Minnesota Department of Human Services requires that an FMS be used for these programs: Consumer Directed Community Supports (CDCS), Consumer Support Grant (CSG), and Community Firest Services and Supports (CFSS). 

What are self-directed services? 

This is a home and community-based service model for people who want to decide what help they need, who will provide the help, and when it will be provided. The person has the choice, authority, responsibility, and flexibility to get customized help with their approved program budget. "You Decide. Your Help".

 What are the participiant's duties when using self-directed programs?

Core duties are to plan and oversee the budget; employ and supervise workers; get other services and make purchases; and follow program rules".

What is Life Fouintain's Monthly FMS fee and who pays for this? 

Life Fountain's monthly FMS fee is $200 when doing payroll and other payment functions for the participant. The fee is included with the program budget for people who are on Minnesota Health Care Program.
